PSG Home Health Care Consumer Directed Services (CDS) consists of personal care (bathing, cleaning, and meal preparation). A participant of CDS must be able to direct the care planning process and hire an attendant. Although similar in nature to in-home services, services are provided to persons with a physical disability and do not include any task performed by a licensed professional. Our goal is to provide individuals more personal autonomy in determining how best to meet all clients’ needs.

To be eligible for the PSG Home Health Care CDS Program you must: 

• Have active Missouri Medicaid
•  Be at least 18 years of age
• Have a physical disability and/or chronic medical condition such as diabetes that  interferes with your ability to care for yourself without assistance
• Have the desire and ability to self-direct your own care and employ and train your own Aide. If you are unable to self-direct or choose not to do so, contact PSG Home Health Care to discuss other options and obtain a referral for other services. PSG Home Health Care works closely with the DHSS to provide joint services if needed. It is possible to receive services from more than one program at the same time or to select which program will best meet your needs.
• If you are currently receiving CDS services from another agency, you will need to contact DHSS to request a transfer of services to PSG Home Health Care. Contact DHSS at 816-889-2206 or email info@health.mo.gov.

We will help you throughout every step of the process!

1. On your first call about the program, we will collect basic information such as your name, contact information, Social Security and Medicaid numbers, etc. We will also ask you to provide a brief overview of your situation and the services you need.
2. You will be contacted by the State to develop your plan of care and to schedule a visit in your home for a review of your current medications and personal care assistance needs.
3. PSG Home Health Care staff will guide you through the process of hiring and supervising your Aide and completing all necessary paperwork.
4. After DHSS approves your care plan, PSG Home Health Care will be assigned and will meet with you to train you about completing and submitting timesheets for your Aide and other program guidelines.
5. Your Aide may be a friend or a family member but CANNOT be your spouse. The Aide must be at least 18 years of age and pass a background screening test.

Our Respite Care Services include: 

• Assistance with activities of daily living
• Performing personal care (bathing, grooming, etc)
• Dressing
• Meal preparation
• Performing light housekeeping
• Laundry
• Providing companionship

Our Personal Care Services include: 

• Assistance with a safe bathing environment
• Personal hygiene assistance
• Diet monitoring
• Meal planning
• Medication reminders
• Bathing assistance
• Assistance with dressing
• Walking assistance
• Assistance with restroom use

PSG Home Health Care In-Home Services (IHS) help people with significant physical disabilities obtain assistance with personal care and activities of daily living to maintain or increase their independence in their homes. This includes services such as meal preparation, light housekeeping, bathing, dressing & grooming, medication set-up and supervision, respite care, advanced personal care, ostomy care, catheter care, bowel program, and transportation.

Medicaid

Medicaid provides health coverage to millions of Americans, including eligible low-income adults, children, pregnant women, elderly adults, and people with disabilities. Medicaid is administered by the state of Missouri, according to federal requirements. The program is funded jointly by the state of Missouri and the federal government.

Private Pay

Your loved one may have savings or supplementary income available that can help pay for home care, assisted living, or nursing home care. Sometimes, family members may contribute to the cost of care for their loved one.

Veteran’s Aid & Attendance Pension Benefit

Eligible veterans and surviving spouses may be eligible for health care expenditures. We can help you evaluate this option to see if your loved ones qualify, and if so, apply for this little-known benefit.
